Herkenning van wekedelenopaciteiten op een panoramische röntgenopname: heterotopische ossificaties en corpora aliena

Regelmatig worden op een panoramische röntgenopname opake structuren gezien die niet in het bot maar in de weke delen zijn gelegen. De differentiële diagnose van dergelijke opaciteiten is divers en bevat vaak voorkomende maar ook zeldzame afwijkingen. Vanwege de diversiteit is het klinisch duiden van een wekedelenopaciteit vaak lastig. Er wordt onderscheid gemaakt in heterotopische calcificaties, heterotopische ossificaties en corpora aliena. Heterotopische calcificaties worden onderverdeeld in 3 groepen, te weten dystrofische, idiopathische en metastatische calcificaties. In dit artikel wordt de nadruk gelegd op de radiologische en klinische kenmerken van heterotopische ossificaties en corpora aliena en hoe deze op een panoramische röntgenopname herkend kunnen worden. Daarnaast zal aandacht worden besteed aan de noodzaak tot eventuele aanvullende beeldvorming en de indicaties voor een mogelijke behandeling.
How to detect opacities in the orofacial soft tissue on a panoramic radiograph: heterotopic ossifications and foreign bodies
Opacities in the orofacial soft tissue rather than the bone are frequently seen on panoramic radiographs. The differential diagnosis of these opacities is diverse and concerns frequently occurring disorders but also rare ones. Due to the diversity of diagnoses, the clinical interpretation of a soft tissue opacity is often difficult. A distinction is made among heterotopic calcifications, heterotopic ossifications, and foreign bodies. Heterotopic calcifications are subdivided into 3 groups, i.e. dystrophic, idiopathic and metastatic calcifications. In this article, stress is placed on the radiographic and clinical characteristics of heterotopic ossifications and foreign bodies, and how they can be recognised on a panoramic radiograph. Besides this, attention will be paid to the need for possible additional imaging techniques and indications regarding treatment option
